Objectives:

Patient safety is a continuous priority in healthcare, which operates within a complex system where multiple failures can contribute to patient harm. Enhancing safety requires understanding that errors often result from a combination of system breakdowns. By implementing strong, effective systems, practices can reduce liability and improve outcomes. This activity will explore common system failures and highlight strategies and processes to minimize practice errors and enhance patient safety.

Identify comprehensive documentation of patient care and other related patient management decisions
Adapt awareness of cultural linguistic competency and implicit bias when interacting with patients and their families

Speaker Bio:

Monica Ludwick, Pharm.D., MBA is the Assistant Vice President of the CAP Risk Management and Patient Safety Department. In addition to her substantial experience in pharmacology and risk management, Monica is actively involved in several professional organizations and has earned outstanding employee recognition multiple times during her career. She earned a Doctor of Pharmacy degree from the University of California San Francisco, School of Pharmacy.
